Troubleshooting

NYBE asks for an issuing agency, but Hasan is a private recipient

This is the signal that CE-200 may be the wrong route. The WCB CE-200 page says the certificate is for attesting to a government entity for a license, permit, or contract, and cannot be used to show another business or that business's insurer that coverage is not required. Ask Hasan whether his compliance team accepts CE-200. If not, move to the carrier/agent certificate route.

Hasan says "workers comp exemption" but does not know the form name

Ask him whether the compliance team needs CE-200 or a carrier/agent workers comp certificate such as C-105.2. Do not assume. Use the clarification message in Decision Gate 1.

NYBE says the application is denied or coverage is required

Stop. Save the denial/notice privately and contact a licensed NYS insurance agent or carrier for the correct workers comp and disability/PFL coverage path. Do not send Hasan an exemption claim after a denial.

Carrier can issue GL but not workers comp proof

Ask Hasan whether GL alone is enough. If not, contact a NYS licensed insurance agent or workers comp carrier. Do not describe the packet as workers comp compliant without the accepted proof.

Hasan asks for W-9 or ST-124
